TARGETED CASE MANAGEMENT

Individuals who are deemed eligible for services through the Division of
Developmental Disabilities are entitled to receive service coordination.
Service Coordinators meet the individuals on a regular basis to assist with the goals outlined in the individuals' person centered plan. Planning, advocacy, linking, and referrals to resources are provided to each program participant. Targeted case management is offered to both Maries and Osage County citizens.
